This form must be completed for all equipment delivery’s, where the equipment contains a hard disk drive or is a data carrying device, such as a laptop, desktop, mobile phone or tablet.

If you want to data erase the device at home (Windows type laptops/ desktops) then please click here. If you buy the software from us and deliver the equipment to us, we will donate the software cost to charity.
